How to Qualify for the swim program

  1. Having a disability is requirement of the swim program
  2. Must be low income showing paper work that can verify monthly or yearly income (income chart on the back of the sheet) Things like bank statement or SSDI paper work to prove income.
  3. Create a goal to best use the pool visits 
  4. Come back to the center for a follow up appointment to track progress of goal set for the pool program.

What to expect with the pool pass program

  • The swim pass is only valid for the year it was received
  • Appointments must be set up ahead of time to enter into program in order to have paper work prepared for the appointment
