How to Qualify for the Swim Program

1. Have a self-identified significant disability
2. Must be low-income 
3. Create a goal to best use the pool visits 

What to expect from the pool pass program

• The swim pass is only valid for the calendar year in which it was received
• Appointment is needed to create a pool pass program file 
• You must come back to the center for a follow-up appointment to track the progress of the set goals
