Community Choice Guide (CCG)
CWDR is contracting with DSHS as a Community Choice Guide (CCG), a Washington State program created with the intention of supporting individuals with maintaining and/or gaining independence through a variety of services. This program aims to support those going through a community transition, provide stabilization for those already in a community living situation, and/or assist with moving into less restrictive community settings.
This is a paid (fee for service) program, only available via referral.
Some of the services offered through this contract include but are not limited to;
Identify and coordinate resources needed for successful integration into the consumer’s chosen community-based setting.
Assist with locating accessible and affordable housing options, including assistance with filling out paperwork.
Help connect the consumer with community activities.
Assist with setting up necessary services when transitioning into a new living situation
Purchasing necessary items for the consumer’s home.
Purchasing regular grocery and self care items.
